UNDATED (WSAU) A Michigan company was fined $2,800 for safety violations that led to the deaths of two workers in Tomahawk last year.
47-year-old Dennis Gougeon and 29-year-old Dustin Hale suffered fatal burns from an ash fire at Packaging Corporation of America in Tomahawk. They were working for Inland Systems, a company that was hired to do maintenance work at the plant. They were using a high-pressure water-sprayer on a fly ash bin when it ignited.
Inland Systems was cited for not following proper safety procedures. Gougeon died the day after the accident. Hale died two weeks later from complications at a Minnesota hospital.
