SCHOFIELD, WI (WSAU-WAOW) — Greenheck Corporation in Schofield has been fined by the Occupational Safety and Health Administration after the death of an employee last July.
WAOW TV reports that the agency has issued a $31,250 penalty against Greenheck for the July 10th, 2023 incident. Investigators found the employee was clearing a jam in a machine when they were caught between two carts.
Additional information shows the machine was not turned before the employee went to clear the jam, causing the second cart to keep moving. The investigation also found that Greenheck “did not ensure that the hazardous energy controls procedures for [the equipment,] including the Operator Safety Control Zones, provided clear and specific guidance for effective means and methods of hazardous energy control during service and maintenance tasks.”
The incident caused serious injuries to the employee’s upper body. They were rushed to a local hospital where they died a few hours later from the injuries.
No further details, including the name of the employee, have been released.
OSHA records show the fine was levied on January 8th.
